Photo I.D. (valid driver’s license, passport, state I.D., etc.), your insurance card, MRI/x-rays/CT (hard copy OR on cd-if available). We do have online access to some local imaging facilities. Please check with the staff.
HOW LONG WILL MY APPOINTMENT BE?
Your time is valuable and important to us. If your forms and records are in order, anywhere from 1 to 1 ½ hour from your check-in time.
